About Registration
- Visit Reception / Helpdesk for new patient registration A Unique Hospital Identity Number will be issued to every newly registered patient.
Admission, Rooms and Prescription
- Patients and their attendants are requested to come after taking appoint of desired doctors or departments. In case of emergency, no prior appointed is needed. You can directly report to emergency room (ER) with patient. It will be wise if reception is informed before your arrival. If you have already consulted, then come with previous prescribed medicine list, conducted diagnostic reports (bring the film in case of ultrasound, X-ray, CT Scan or MRI) prescribed by other consultants, hospitals or nursing homes. It is also informed all of you that room charges may vary as per their categories. There are three categories of room, General Ward, Semi-private Delux.
- It is requested to the patient to accompany by members of family, relatives or friend to ensure smooth coordination and updating about health status.
- The patient companion must be 18 years old or above and in good health.
- Prior admission, the patient/attendant will be asked to sign the consent form available at Helpdesk/Reception.
- Prior to admitting the patient, Option of room and their availability will be given to you to choose and its cost will be charged accordingly.
- Before admitting the patient on a bed, the bed will be cleaned and disinfected carefully by the housekeeping staff and it will be supervised by their immediate supervisor.
- During the stay, the room and bed be cleaned and disinfected by housekeeping staff and it will be supervised by their immediate supervisor.
- In case of admitting the patient, an identification band/Medical Record Number (MRN) will be issued by our medical staff.
- Patients are requested to wear and display the Identification band provided by the ICU team.
- In case of emergency, patient belongings will be kept by their attendants.
- The patient/their attendant will sign an agreement that cost of treatment will be paid at regular interval whenever asked by account and its team.
- Planning for discharge is equally important as admission. During admission, our team will be entirely responsible for your care and they will plan for your discharge after coordinating the doctors and other responsible teams. Every patient will be informed about their discharge, it date and time after getting clear from care and management teams.
- Once the patient will be informed about their discharge, the patients/their attendant must inform about the same before an hour.
- You requested to clear all dues and collect necessary document before leaving the hospital.
- Before discharging the patient from the hospital, management/ or concerned must ensure that all necessary instructions are given to the patients.
- We at Medaz heartedly welcome the visitors of the patient because we believe that they not only boost the morale of the patient, but also play a crucial role in their speedy recovery.
- Visiting hours for General Ward will be 11:00 AM to 8:00 PM.
- Only one / two persons will be allowed at a time to avoid crowding in the General ward for not more than 30 minutes.
- Only one attendant will be allowed to stay with a patient at the night in general ward remaining relatives and friend may wait in waiting hall.
- Visiting hours for ICU will be 11:00 AM to 3:00 PM.
- In order to avoid the infection, only one person will be allowed at a time and it will be only for just 10 minutes.
- To avoid crowding in ward, all attendants are requested to wait for their turn.
- You are advised not to visit, if the patient is sick.
- In general ward and ICU, please keep calm.
- Always keep moral high of the patients.
- Attendants of all patients are requested to do not carry any eatable products in the general ward and ICU as it may cause infection to patients.
- Attendants of patient are requested to wear isolation gown, mask, gloves, shoe covers and use hand sanitizer before enter in the ICU room. These are available at ICU management desk
- All staffs have strictly been instructed to adhere to the guidelines of safety and security procedure of the hospital.
- Leave the hospital immediately, once you hear the fire alarm.
- In case of Fire and Earth Quake, every patient of hospital will be evacuated as per the safety plan made by Safety Management Department of Medaz Hospital.
- In case of earthquake use only stairs instead of lift.
- Personal privacy of the patient must be protected.
- Physically disabled patient should be given reasonable helps.
- Must be given equal treatment to every patient without discriminating on the base of economic status.
- Must be given medical detail about every patient and their information must be protected from the misuse.
- They must be given adequate treatment in safe and secure atmosphere.
- Infants, small children, disabled individuals and the elderly patient must be protected from abuse and physical assault.
- All kinds of information like diagnosis, treatment, benefits of patients, and potential communication, possible options of treatment must be communicated in the simplest language that should be easy to understand.
- Must be consented before taking picture of face of the patient.
- If the patient refuses from diagnostic procedure, they must be informed about its consequences.
- They have right to know about the person involved in treatment, diagnosis and other services.
- Request of patient must be listen if they wish take second opinion.
- They must be informed if they are being transferred from one consultant to another consultant.
- They must be informed if their treatment is for experimental research. In that case a written consent is mandatory.
- Their consent must be taken in case of tissue/organ donation and receiving.
- They must be explained about financial statement in the simplest way.
- Patients and their attendant should respect the policies of hospital, staffs, care for its property, and also the right of other patients.
- All patients and their attendants must carry and eat only vegetarian food in cafeteria.
- Eatables are not allowed at OPD section, Reception/Helpdesk, OT area, and Emergency section.
- Patients are requested to do not take any eatables for patients from outside in hospital it may cause infection.
- No one is permitted to smoke in hospital premises.
- For better treatment and diagnosis, patient is requested to give accurate information about medical history, symptoms, present medication, and other information without hesitating. We will protect your privacy in every situation.
- Inquiring about patient’s health status and improvement.
- Must ask about the consequences in case they do not follow the words of practitioners.
- Informing the consultant about the improvement especially in pain and symptoms of patients.
- Informing about the safety and security of patients.
- Unnecessary interference must be avoided
- Any kinds of harassment including unnecessary calls to staffs, especially female staffs.
- Use of unofficial languages, shouting, bullying and indecent language/gestures.
- No one allowed entering in hospital premises under the influence of any kinds of ban drugs, alcohol, and liquor.
- Any kinds of damage to property of hospital are punishable offense.
- Shoplifting/mugging.
- Making noise or disturbing the other patients.
- Acceptance of money in exchange of any kinds of favour is punishable offense.
- No staffs are allowed to accept any kinds of gifts, money or token (shopping voucher) from any medicine’s companies, patients or their attendants. You may be terminated for such activities.
- Any kinds of intimacy with colleague are unacceptable. You are requested to maintain relationship outside the premises of hospital; You may be terminated for such activities
